As a Federally Qualified Health Center Look-Alike (FQHCLA), Damar Health Services complies with HRSA requirements and functions as a true safety-net provider. This means we work closely with other healthcare providers, ensuring your care is coordinated from start to finish. From checking whether you have transportation to making sure your appointment details and follow-up information are recorded in your chart, our focus is on delivering full, continuous care. Being an FQHCLA also means we provide comprehensive primary services, use a sliding fee scale, and offer care regardless of your ability to pay – so you receive accessible, accountable, and community-centered health care.
Because we believe that quality services should be available and accessible to all, regardless of an individual’s circumstances, Damar Health Services provides innovative and effective healthcare services in communities of greatest need. The services we provide includes:
Screenings, lab work, and on-going education.
Refill a prescription or manage medications online.
Immunizations, growth and development, monitoring and on-going education.
Screening, evaluation, monitoring and on-going coordination.
Screening, evaluation, and individualized care planning.
Coordinated care with your other providers, insurance navigation, and referrals.
Required physicals for youth sports league participation.
Evaluation and treatment for minor injuries, illnesses, and wellness exams.
We accept Medicaid, Medicare, and private insurance. If you’re uninsured or under-insured, we offer a sliding fee scale – and no one is turned away because of inability to pay. That’s part of our commitment as an FQHCLA to expand access to care in our community.
