New patients may call for an appointment at (317) 455-2366 or schedule online by providing name, email and a phone number. You will then receive an alert to schedule at a time convenient for you.

Fee Schedule

As a community health clinic, Damar Health Services provides care on a sliding fee scale. Patients will be seen regardless of their ability to pay. We accept Medicaid and private insurance, as well.
